Heartland Functional Medicine accepts some COMMERCIAL insurances, including:

Blue Cross Blue Shield
Midlands Choice
United Healthcare including: Allsavers, Golden Rule, UMR—Closed to New Patients
United Healthcare Community Plan—Closed to New Patients

Appointments are available in person and by phone or video conference.

Clients are responsible for copays, coinsurance, and/or deductible amounts at the time of service. We accept Cash, Check, Visa, Mastercard, Discover, and American Express. If we are not part of your plan, we collect in full for services rendered.
