Once scheduled, you will receive a welcome email along with an online intake from. It is important to have this filled out prior to your appointment date. It can take about an hour to fill out. The information you share for your first appointment will be shared with you again for any follow-up appointments as well, keeping us all on the same page on your journey to health and wellness.
